Legacy Workbooks

Turnkey tools to engage on your own

The Care Partner Workbook and the corresponding Loved One Legacy Book use elements of dementia education, Music Therapy, Art Therapy, self care, and legacy work. Easy and engaging exercises guide you through creative ways to express yourself, get support, and stay connected along the dementia journey. Discover new tools to support your loved one living with dementia to tell the story of who they are, leaving you with a personalized Legacy Project that you’ve accomplished together!

The two Workbooks support you and your loved one in different ways because dementia affects people in different ways; but just like us, they work better when they work together.

Engagement Plans

Tailored sessions designed just for you, your loved one, and your care community

Step 1: Introduction

No two families touched by dementia are alike, so the Collective Team wants to know about you, your goals, what’s important to you, and how dementia has touched your lives. 

Step 2: Your Engagement Plan

We tailor your Engagement Plan with either four, eight, or twelve sessions to help you meet your goals, and then some! As always, we use elements of dementia education, Music Therapy, Art Therapy, Pet Therapy, self care, and legacy work. Using one primary modality or a co-treatment approach, we coach you to master new tools that support common goals, such as: 

  • Knowing what’s changing v. what’s retained (and how to leverage what’s retained)

  • Communication (even when formal language is failing!) 

  • Skills to promote independence

  • Redirection & navigating sundowning

  • Non-pharmaceutical pain management

  • Creating daily routine, structure, and purpose

  • Fighting isolation by effectively getting other people to help 

  • Being remembered the way you want to be remembered

  • Self-care (it’s not just a buzzword)

Step 3: Include your Care Community

Different sessions may include Care Partners (you), your loved one with dementia, or other members of your Care Community. We complete your Engagement Plan with a final Legacy Project debut and community call to action!
